Transcript Request

Parchment request for high school transcript:

If the Parchment request does not work, please send an email request with the below information to Ashley.Baker@CarlisleIndians.org.  The school is not required to keep copies of high school diplomas per the Ohio Department of Education. Transcripts are accepted as proof of education.

  • First and Last name while attending Carlisle High School
    •    Current last name if it has changed
  • Birth date
  • Graduation year or last year of attendance
  • The method you would like to receive your transcript   
    • Email - provide email address
    • Mail - provide complete mailing address
    • Pick up in person - provide current phone number to receive a call when it's ready (this may not be an option during summer break)
    • Fax - provide fax number (this may not be an option during summer break)
